Faith school admissions and supplementary information forms 

Faith schools often give priority to children of faith. 

These schools ask for more information about the family’s faith on a form called a supplementary information form. If you’re applying for any of the faith schools listed, you need to fill in their supplementary form as well as your school application. 

You can find the form on the school website or contact the school to ask for a copy. 

You must send the form back to the school by the application deadline.
